Histoplasmosis is a systemic fungal infection caused by H. capsulatum usually in immunocompromised individuals such as those with HIV-AIDS. Though oral manifestations are rare, they could be the only and initial manifestation of We report case 23-year-old female who presented ulcer palatal perforation detected to suffering from disseminated histoplasmosis. She also turned out HIV positive. Her CD4+ count was undetectable. responded well intravenous liposomal Amphotericin B, itraconazole antiretroviral therapy. To best our knowledge, histoplasmosis presenting first HIV-AIDS has not been reported South India previously. A high index suspicion needed cases exaggerated lesions even states like Kerala avoid delay diagnosis
